Residential Counselor
at PATH, Inc. in Philadelphia, PA
Responsible for assuring people receive all of the supports they require as individuals and as a group of persons sharing a home. Levels of individual supports may range from intermittent to pervasive supports.
EMPLOYEE TRAITS:
A. Interested and motivated to assist in the creation and maintenance of progressive learning environments as developed through the team process, which enhances an individual’s quality of life.
B. Interested and motivated to assist individuals to create friendly and warm relationships with neighbors and their immediate community.
C. Interested and motivated to support individuals in maintaining a comfortable, safe and pleasant home.
PHYSICAL DEMANDS:
Work is active. Performed within the individuals’ home, as well as in the community. A variety of lifting and transfer supports for individuals may be required to ensure individuals’ mobility.
SPECIFIC DUTIES:
1. Support individual by providing assistance and training in all areas of daily living,
including, but not limited to, the following:
a. Self-care, grooming, hygiene
b. Care of clothing, laundry
c. Cooking, nutrition, dietary needs, meal preparation, menu development, grocery shopping
d. Household maintenance, cleaning
e. Budgeting, banking, comparative buying, financial documentation
f. Leisure activities, recreation
g. Appropriate social behavior
h. Utilizing various modes of transportation for community integrated activities
i. Awareness and use of community resources
2. Participate in the individual’s program planning process by:
a. Attending and contributing to annual and ancillary meetings, as scheduled
b. Assuring individuals’ rights consistent with agency policy
c. Implementing program goals
d. Documenting goal progress
e. Completing monthly reports
f. Supporting individuals’ independence as defined in their program plans
3. Assists individuals in maintaining good health by:
a. Assuming knowledge of an individual’s medical history and current health issues
b. Assuming knowledge of an individual’s physician and ancillary health professionals
c. Accompanying an individual to medical appointments or emergency service, as necessary
d. Communicating with supervisory staff about the health needs of an individual
4. Assures individuals’ safety and a safe home by:
a. Teaching safety precautions in the home and community
(1) Conducting monthly fire drills, which includes checking the fire extinguishers
(2) Checking smoke alarms daily
(3) Submitting repair request forms for routine maintenance items
(4) Notifying supervisory staff of emergency safety issues
5. Address various individual needs by:
a. Communicating with co-workers and the appropriate supervisor
b. Reviewing co-workers’ log entries
c. Documenting daily in individual communication logs
d. Relating to families, guardians, advocates, neighbors and other program staff in a professional manner
e. Notifying the on-call supervisor of emergencies via team beeper
6. Supports and assists individuals in maintaining their home by involving the individual in:
a. Shopping for food and other household purchases
b. Preparing meals
c. Managing weekly household expenses
d. Weekly household cleaning
7. Acquire a minimum of 24 hours of training annually and within required time frames, including, but not limited to:
a. Medication Administration
b. Fire Safety
c. CPR
d. First Aid
e. Blood borne Pathogens
f. Crisis Intervention
8. Other related duties as required.
Requirements QUALIFICATIONS:
Minimum requirement of High School diploma; any relevant certification, college, or combination of above is considered a plus. Need to be respectful, sensitive, caring and patient. Knowledge of and commitment to the philosophy and values in supporting individuals to maximize their independence and potential. Demonstrate good oral and written communication, organization, planning and problem-solving skills. Also, need to show initiative and be self-motivated. Must be able to respond expediently to emergency situations. Must be able to physically lift and transfer individuals as required.
Physical examination is required, including TB test by the intracutaneous mantoux method no more than twelve months prior to employment and every two years thereafter. A valid driver’s license and satisfactory driving record is required for certain positions.
